NOTE: As of June 24, 2020 we now have ONLINE FORM SUBMISSION! So … you don’t have to print out the forms and bring them with you – you just have to fill out the form online and click SUBMIT.
We strongly urge you to use the ONLINE FORM SUBMISSION. We have removed the old option of printing out the forms for completion with a pen. We hope this will help with the problem of forgetting to bring the forms to your appointment.
